When seating the bearing and then the lock washer and axel nut I noticed a space between the bearing and lock washer approximately 1/4 inch. Is this normal?

The lock plate has a shoulder on it that should face down during installation. This should provide a small gap between the bearing and the lock plate so that you will be able to pry the plate up and into the slots on the nut. This gap shouldn't be anywhere near 1/4 inch though.

Yes as Dan explains, too much gap. Sounds like the lock washer hung-up during installation.
