How do you install the lower control arm with the end caps having threads on both the outside and inside?

First remove both caps to install or remove the control arm. Once both caps are off, the control arm can be installed or removed by slipping the A arm onto its mounting pivot on one side at a time. Once both sides are hanging from the pivot, the caps can be threaded on. Keep the A arm spacing equal on each side as the caps are screwed on.

I think you are asking a similar question to me.  I was going to make up some simple spacers so I can ensure mine are equal spacing at both ends.
