1965 250SE Coupe W111
You can see the 2 bolt holes where the bracket is missing from. I think the correct part has some rubber grommets in there and attach to only one of the exhaust pipes. I just can't find the part number for it.
There is also a picture here of what I believe it should look like.

It's not as simple as just finding and installing the bracket and a few bushings. There is a plethora of parts that make all of this up.
The bracket that goes from the exhaust pipe to the transmission is 108-492-03-19. Still available and very affordable.
The bushings actually came in a kit. It included the "U" shaped clamp, washers, spacers, bushings, etc. Part number 126-490-03-37. Unfortunately, that kit is no longer available, but the individual parts that make up the kit should still be available separately. This list of parts are as follows:
123-492-00-27 "U" shaped clamp
(2) 000-990-32-50 Copper nuts for "U" shaped clamp
( 8 ) 002093-016100 Washers for "U" shaped clamp copper nuts (4 on each side)
(4) 121-155-01-81 Eslatomer bushings
( 8 ) 000440-009103 Metal washers for elastomer bushings
(2) 126-492-00-76 Washers for between bracket and transmission
(2) 000931-008266 Screws for bracket, bushings, and washers through bracket and transmission bracket
(2) 126-492-01-72 Spacer tubes for in between bushings
(2) 304032-008005 Nuts for through screws
If that is too daunting for you then you can order kit 126-490-04-37, which is still available, but will include a few items you don't need.
You also seem to be missing a heat shield that goes between the transmission and where this exhaust bracket mounts to. It shields the modulator valve diaphragm from exhaust heat. See picture below. Part number is 111-492-04-30. It's no longer available, but the part number will be a good reference.

I have the bracket and the kit and I'm thinking the holes in the bracket need to be a fair bit bigger. Am I missing something?

The rubber bushings/spacers attach to the bracket on the transmission first, then the bracket shown in photo is bolted through the bushings and bracket on the transmission. Part number says it was originally for a w126 chassis vehicle, but looks like it may be the factory substitute for earlier cars also.

Is this the correct location for me to attach the heat shield?
It's only loosely jammed in there.

That looks about right. There is a clamp to hold the shield on the trans filler tube. BTW, one of the wires for the pressure switch also goes through this clamp. The other end is held by a screw and nut about where you have it.

Thanks for helping. Can you confirm that this is the pipe it clamps to (see picture)?
Do you know where it clamps at the back (see the other picture)?
My shield I think could be twisted so it really hard to work out where it is supposed to go
