Application for
Full Paid Membership in the
international Pagoda SL Group
This information will be used by the Pagoda SL Group to enroll the applicant as a
Full Paid Member. The Pagoda SL Group does not sell this
information to any other organization.
This application is not effective until the proper
payment has been received and the membership application
has been approved by the Pagoda SL Group. Membership conditions
are governed by the Group Charter as may be amended
from time to time. A copy of the Charter is available
for review Group Charter
Annual Dues are 30 US Dollars or 25 Euros. The dues cover the 12 month
period following receipt of the payment.
Upon receipt of the proper dues and upon approval
of the application, a Member Number will be assigned
to the applicant and access to all parts of the Pagoda SL Group
website will be activated.
This application can be filled in and transmitted
to the Group with the dues payment by clicking the
Paypal box below or this form can be printed out and
mailed with the payment to the Pagoda SL Group to one of
the addresses below.
